This privacy policy tells you what to expect us to do with your personal data.

Personal data received by us via the website, email, social media or in person, will be used for the purposes specified in this policy, in order to fulfil our obligations to you as our client. We will not provide your personal information to any third parties without your consent.

What information do we collect?

We collect and use the following data in order to provide care and services. This may be processed and transmitted electronically, on paper or both.

  • Name, date of birth, address, telephone number, email address and general medical practitioner details

  • Information about care and services provided or proposed

  • Records of consent to treatment

  • Health information

  • Relevant information from other health professionals (including test results)

  • Any contact we have had with you including appointments booked and Pilates classes attended

  • Information regarding payment method and payments received (please note we do not ask for or store credit or debit card details)

We may gather information such as: name, address, telephone number, and email address when you visit and interact with our website. We can collect your IP address, information about what pages you viewed on our site, and how much time you spent on our site.

It is important that you tell us if any of the details we hold about you have changed or are incorrect. You have a responsibility to inform us of any changes so our records are accurate and up to date for you.

Why do we hold information about you?

 We need to keep comprehensive and accurate personal data about clients to provide you with safe and appropriate care or services. We use the information in the following ways:

  • For the basis of appropriate care

  • To provide information or personalised exercise plans

  • Invoicing for services

  • Communication, including appointment reminders via email, SMS or WhatsApp

  • For clinical audit to monitor the quality of the service provided. In this case some data may be held centrally and used for statistical purposes. When we do this, we ensure that patient records cannot be identified.

Data retention

We will retain your treatment records for a maximum of 7 years or, in the case of minors, until the individual reaches 21 years of age (whichever of these is longest).

Security of information

Personal data about you is held in a password protected computer system and/or in a locked manual filing system. The information is only accessible to authorised team members. Our computer system has secure audit trails and we back up information routinely.

Access to your records

You have the right to ask us to provide you with the information we hold about you and to have any inaccuracies corrected. If you would like a copy, please email swiftalignrehab@gmail.com. There may be a charge for this service.

We may disclose your personal information to the following only if we are required to do so by law and/ or you have provided consent either implicitly for your care, or explicitly for other purposes:

  • Health care providers

  • Organisations we need to share information with for safeguarding reasons

  • Professional or legal advisors

  • Relevant regulatory authorities

  • External auditors or inspectors

  • Other relevant third parties: such as our third party booking system

If we contact your GP, consultant or other healthcare provider, we would first seek your verbal consent to do so.

 

Access to your personal data is provided to carefully selected third parties. We use third-party data processors to help us run and deliver our services. You can read more about our third-party data processors when you directly use their services (i.e. video consultation platform, online booking platform etc.) by referring to their own privacy policies.  

Your rights

If you do not wish personal data that we hold about you to be disclosed or used in the way that is described in this policy, please discuss the matter with us. You have the right to object and withdraw consent; however, this may affect our ability to provide you with our service.
